Keratosis lichenoides chronica (also known as Nekam’s disease)

  • Violaceous, keratotic, lichenoid papules in a symmetrical, linear and reticulated pattern on limbs and trunk 
  • Greasy and scaly lesions or psoriasiform scaly plaques on the upper face + keratotic papules on palms and soles
  • ± nail and scalp involvement
  • Chronic and progressive course, but some lesions can regress (summer and aging)

  • Similar histologic and immunofluorescence findings as lichen planus

Symptomatic treatment (usually unsatisfactory): 

  • Topical corticosteroids
  • Systemic corticosteroids
  • Methotrexate, cyclosporine A
  • Etretinate
  • Oral vitamin A
  • Psoralen and ultraviolet A/Narrowband ultraviolet B
