Amphotericin B  (polyene)

Regular

  • 1 mg/kg/day

Liposomal

  • 3-5 mg/kg/day; adjust with creatinine clearance

  • Binds ergosterol & forms pores in fungal cell membrane

  • Almost any severe invasive fungal infection; also used in leishmaniasis

  • Fever, chills, nausea, vomiting, seizures
  • Nephrotoxicity
  • Hepatotoxicity
  • Arrhythmias 
  • Agranulocytosis
  • Hypersensitivity, fixed drug eruption

  • Caution when concomitant treatments that lower potassium & are nephrotoxic; as well as those that increase QT interval
