Spitzoid melanoma – low magnification

LEGEND

This is a melanoma that clinically and histologically resembles a Spitz nevus.

Numerous melanin granules in the dermis distributed asymmetrically

  • An asymmetrical, poorly defined lesion is observed at low magnification, with mild epidermal hyperplasia. The nests vary in size and shape and are not equidistant.

  • Dense band-like lymphocytic infiltrate beneath the melanocytic lesion

LEGEND

Vertically oriented nests composed of large, atypical spindle-shaped and/or epithelioid melanocytes

  • Frequent retraction artifact around nests

  • Absence of maturation, meaning that the melanocytes in the dermis are as large and atypical as those at the dermoepidermal junction

  • Presence of Kamino bodies

  • Marked pleomorphism. Atypical spindle and epithelioid cells of highly variable sizes and shapes are seen.

  • Presence of mitoses in the dermis
