Compound acral nevus

Absence of atypia and mitoses.

Normal maturation

  • The presence of thick compact hyperkeratosis is a good clue for acral location

  • Lentiginous and nested proliferation of melanocytic cells in the epidermis and dermis. This is a symmetrical lesion without a shouldering phenomenon (see dysplastic nevus)

Compound acral nevus

Melan-A highlights the melanocytic proliferation

In one-third of cases, pagetoid ascent can be observed in the center of the lesion. This is not worrisome for melanoma.
