Contrary to what the name suggests, this is not a granuloma; rather, it is classified among vasculitic and eosinophilic disorders

Epidermis spared

Histopathological features of leukocytoclastic vasculitis may theoretically be present, but they can be very subtle or even absent, as in the presented case

  • Marked and characteristic Grenz zone

  • Dense, mixed inflammatory infiltrate in the dermis composed of lymphocytes, histiocytes, neutrophils, plasma cells, and eosinophils

  • Characteristic Grenz zone (band of spared papillary dermis separating the epidermis from the underlying dense dermal infiltrate)

  • Marked solar elastosis (degenerated bluish material located in the papillary dermis)

  • Dense, mixed inflammatory infiltrate located beneath the Grenz zone
